We know your time is valuable. In order to decrease your wait in our office, you can now complete most of your pre-exam forms online. If you are a current patient you will need to contact our office for your passcode.
In addition to completing your online paperwork, please bring a list of all current medications including dosages and how often you take them with you to your visit.
Remember to inform our front desk staff you have completed your forms online when you arrive for your visit.

We make every attempt to maximize your insurance benefits to save you money. To facilitate this, please bring a photo ID and both your medical and vision insurance cards. If you are not the policy holder, please provide us with their full name and date of birth when you arrive.
